You have two ways to keep your life insurance coverage, and one way to retain your disability coverage. Click on one of the options below to get started.

Application Deadline Reminder

To be considered for portability or conversion of coverage, OneAmerica must receive your completed and signed application and required premium within 31 days of termination of coverage under your employer’s group insurance.


Keep My Life Insurance Coverage through Portability

Portability allows you to continue the life insurance coverage as offered by your employer. This option is available to everyone under age 70 and may allow you to continue coverage of eligible dependents if you continue your coverage.

Note: Premiums are not guaranteed and may change at any time. The certificate does not accrue cash value.

3 STEPS to apply for group term life portability

You will need the following information to submit with your application:
  • Employer and group insurance information (from your employer)
  • Bank draft information to set up automatic payment
  • Beneficiary information, and dependent information if you are continuing dependent coverage

Keep My Life Insurance Coverage through Conversion

Conversion allows you to turn the life insurance coverage offered by your employer into an individual whole life policy. This option is available to everyone under age 85 and may allow you to continue coverage of eligible dependents, even if you opt out of coverage for yourself.

Note: Premiums may be different than the group policy but are guaranteed not to increase once the individual policy is issued. Whole life insurance policies accrue cash value and have the potential to earn dividends.

You will need the following information to submit with your application:
  • Employer and group insurance information (from your employer)
  • Bank draft information to set up automatic payment
  • Beneficiary information, and dependent information if you are continuing dependent coverage

Keep My Disability Insurance Coverage

Portability allows you to continue your group disability insurance offered by your employer for up to 12 months after a change in employment status. Coverage is available to individuals under age 70 who are not receiving disability benefits and meet all other eligibility requirements.

3 STEPS to apply for portability of group disability insurance

To fill out the employee section of the application, you’ll need to provide your personal information, along with some employer details.
  • If you do not know the Policyholder Number, write down your employer’s name only.
  • Sign and date only Page 2 of the application

Your HR administrator is responsible for completing Page 3 and submitting the full application to OneAmerica within the 31-day deadline.

While you cannot submit the application yourself, you can set up reminders to check in with your HR administrator and make sure your application is submitted on time.
